Please follow these steps to become a new client:

1.     Call 763/218-3231 to schedule an appointment or consultation date and time.

2.     Complete the registration form below to register as a new client and confirm your appointment.
         (If you do not have an appointment, please first call 763/218-3231.)  Your appointment is not
         confirmed until you complete this step. Email addresses are kept completely private.
         All new clients are added to the ChangeWorks Hypnosis Monthly newsletter and you may 
         unsubscribe at any time.



3.     After submitting the form, please download and fill out the appropriate form(s) as indicated 
         below, and bring these to your first appointment.

         •  All clients:  complete Client Bill of Rights and TMBC Intake Form
         •  Smoking clients:  Complete the Tobacco Use Questionnaire

You may pay with cash, check, major credit card or PayPal.  Full payment for sessions and session packages is required prior to or on the first session, unless otherwise agreed upon. 

Our office is located in Apple Valley, Minnesota.  A link to maps and directions will be provided in your appointment confirmation email, which you will receive automatically after completing the registration form.

4.      Please arrive a few minutes early so we can spend the most amount of time together and you
          can receive the maximum benefit. Out of respect for others, your appointment time is for the
          allotted time discussed.  Tardiness results in a shorter session for you.  This time has been set
          aside especially for you.

5.      Please read the Terms and Conditions below.  By completing the registration form and
          pressing the Submit button, you are agreeing to these Terms and Conditions. 
         
Terms and Conditions

• Cancellations must be made at least eight business hours prior to appointment time. 
• Cancellations, or rescheduled appointments, made with less than eight business hours notice, will be billed as an appointment, or count as an appointment in your package.
• Tardiness of more than 15 minutes results in cancellation of appointment, unless otherwise agreed upon. 
• Missed appointments are subject to a charge of 100%of session cost, and will be billed accordingly.
• Cancellation of packages: Any used sessions will count as prorated session price. There is a  $30 cancellation fee; you will be refunded the difference.
• If you are under the care of a psychiatrist, I require written approval for your therapy.
• No children unless he/she is a client.
• All sessions may be recorded, audio, and/or video, for archival purposes.  No recordings will be used for public display unless otherwise agreed upon.
• I reserve the right to refuse service to anyone.
